In the radiology OSCE case 418, the artifact depicted is the Magic angle effect, which causes increased signal in the supraspinatus tendon near its insertion site on a coronal image of the right shoulder. This artifact occurs when tendons are aligned at 55 degrees to the magnetic field, leading to focal signal increase on short echo time images. It can be differentiated from pathology by its disappearance on T2 weighted images with longer echo times and the absence of secondary signs of tendon injury. Other sites of occurrence include the intravascular long head of biceps brachii, ankle, and wrist tendons.
