Rice grain calcification represents multiple small, ovoid or elongated calcific densities in soft tissues, characteristically parallel to the long axis of muscle fibers, seen in cysticercosis due to infection with the larval stage (cysticerci) of Taenia solium. It is a minor diagnostic criterion for neurocysticercosis and may be associated with musculoskeletal symptoms like myalgia, muscle hypertrophy, or palpable nodules; neurological complications occur if cysts involve the central nervous system.

Why is it called so?:

The sign is named for its appearance as multiple small, ovoid flecks of calcification resembling individual grains of rice scattered within soft tissues, particularly along muscle fibers.

Pathophysiology:

Ingestion of Taenia solium eggs leads to dissemination of cysticerci to soft tissues and muscles; host inflammatory response kills the larvae, inducing granulomatous reaction followed by dystrophic calcification of the degenerated cysts, resulting in rice grain-like foci aligned parallel to the muscle long axis.
