A bone-within-a-bone appearance is a smaller, denser replica of a bone outlined inside the contours of the parent bone, most often seen in the vertebral bodies and the tubular bones. It records an episode of arrested and then resumed bone growth, the dense line marking the position of the periosteal or endosteal surface at the time growth stopped. Causes include osteopetrosis, growth-arrest (Harris) lines from any severe systemic illness, thalassaemia, sickle cell disease, Gaucher disease, rickets after treatment, hypervitaminosis D, and heavy metal poisoning with lead or bismuth. It is also a normal transient finding in the thoracic and lumbar vertebrae of neonates. It should be distinguished from sandwich vertebra, in which sharply defined dense bands lie along both endplates in osteopetrosis, and from rugger jersey spine, in which the endplate sclerosis is ill defined and blends into the vertebral body, typical of renal osteodystrophy. Paget disease instead expands the bone and thickens the trabeculae, giving the picture frame vertebra.

Pathophysiology
During an interval of illness, toxicity or metabolic arrest, longitudinal and appositional growth slows or stops while mineralisation of the existing surface continues, so a dense layer is laid down at the current cortical or endosteal margin. When normal growth resumes, new bone is added outside that layer, which is thereby buried within the enlarging bone and preserved as a discrete dense line reproducing the shape the bone had at the time. In osteopetrosis the mechanism differs slightly: defective osteoclastic resorption prevents the primary spongiosa from being remodelled, so successive fronts of unresorbed dense bone accumulate as concentric outlines.
