What causes breast in a breast sign in the breast on mammography/US?

Well-circumscribed, encapsulated mass with homogeneous fat density and often a thin radiolucent capsule or halo, characteristic of breast hamartoma (also known as fibroadenolipoma), a benign tumor-like malformation containing variable proportions of normal breast tissue components including adipose, glandular, and fibrous elements.

Why is it called so?

The breast in a breast sign describes the mammographic appearance of a smaller breast-like structure (with fat centrally and glandular tissue peripherally mimicking normal breast architecture) contained within the surrounding breast parenchyma.

Pathophysiology

Disorganized but benign overgrowth of mature adipose, glandular, and fibrous breast tissues forms a well-defined mass; the fatty core appears radiolucent centrally while the fibrous/glandular rim creates a thin capsule-like border, producing the characteristic nested “breast within breast” configuration on imaging.
